Insurance

We are in network with most major insurance carriers.  For patients, out of network or cash pay, we are here to help you navigate any questions surrounding insurance reimbursement for an out of network provider. We do not accept Medicare/Medicaid.

New Patients

Dr. Smith is currently taking new patients. Please call our office to learn more about our practice and our membership model. We are happy to assist you in making the appointment that is right for you. Click here to download, print, and fill out your new patient forms.

Established Patients

We are happy to offer our patient portal to assist you with any needs at Charleston House, such as scheduling appointments, prescription refills and medical questions. You may also call the office directly for any assistance.
